Heartflow, Inc. (NASDAQ: HTFL) experienced a remarkable surge in its stock price on Friday, climbing 34.1% in late-morning trading. The dramatic move followed the release of the company's second-quarter financial results, which exceeded analyst expectations, and a significant upward revision to its full-year revenue guidance. The market capitalization of the cardiac imaging software firm increased by approximately $912 million, reflecting strong investor confidence in the company's growth trajectory.

The company, which specializes in non-invasive coronary artery disease diagnostics, reported second-quarter revenue of $64.1 million, a 48% increase compared to the same period last year. This figure surpassed the consensus estimate of $56.6 million by 13.1%. On a non-GAAP basis, the company posted a loss per share of $0.07, which was narrower than the anticipated loss of $0.13 per share. The improved performance was driven by robust demand for its FFRCT analysis and plaque assessment products.

Heartflow also reported a GAAP gross margin of 83.0%, up from 75.5% in the prior year, and reduced its non-GAAP operating loss to $7.9 million from $11.5 million. The company's cash and investments stood at $246.8 million at the end of the quarter, down slightly from $254.9 million in the previous quarter.

The most notable development was the company's updated 2026 revenue forecast, which now ranges from $246 million to $250 million, up from the previous guidance of $228 million to $232 million. The revised midpoint of $248 million represents a 12.7% increase over the initial midpoint provided in March and implies year-over-year growth of approximately 41%. This marks the second upward revision this year, following a first-quarter performance that prompted a $10 million increase in May.

Investors reacted strongly to the magnitude of the guidance hike, which was described by William Blair analyst Brandon Vazquez as "two times larger than the beat." The $18 million increase in the midpoint came after revenue exceeded consensus by $7.4 million, suggesting that the company's growth momentum is accelerating. The market's response translated into a valuation multiple of roughly 51 times the incremental guidance, indicating that investors are pricing in significant future potential.

Chief Executive John Farquhar highlighted the rapid emergence of Heartflow Plaque as a "meaningful second growth engine," complementing the core FFRCT analysis business. The company serves over 1,800 institutions and has processed data from more than 750,000 patients, underscoring its established market presence and scalability.

Following the surge, Heartflow's shares reached a session high of $42.95, matching its 52-week peak. The stock was trading at $41.59, representing a 4.9% premium to the average analyst price target of $39.63. Despite the strong performance, the stock's valuation remains elevated, trading at approximately 14.5 times the guided sales midpoint, up from 11.6 times before the results.

Analyst sentiment remains largely positive, with eight out of nine covering analysts maintaining a Buy rating. Notable price targets include $45 from Piper Sandler, JPMorgan, and Stifel, while Morgan Stanley holds a Hold rating with a $35 target. However, the highest target of $45 implies only an 8.2% upside from Friday's late-morning price, suggesting that much of the good news may already be priced in.

Despite the optimistic outlook, Heartflow has yet to achieve profitability. The GAAP operating loss widened to $17.9 million from $13.7 million in the prior year, although the adjusted loss narrowed. The company's high valuation and reliance on hospital adoption, reimbursement policies, and regulatory approvals present ongoing risks. Any delays in these areas could pressure the stock, which now trades at a premium multiple relative to its revenue base.

As Heartflow continues to expand its product offerings and capitalize on the growing demand for non-invasive cardiac diagnostics, investors will be watching closely to see if the company can sustain its growth trajectory and eventually reach profitability. The strong second-quarter performance and upward guidance revision have certainly set a positive tone for the remainder of the year.
