CVS Health (NYSE:CVS) delivered a robust second-quarter performance, handily beating analyst expectations and prompting the company to lift its full-year guidance. The stock was indicated to open about 4% higher in premarket trading, though early pricing remained tentative.

Adjusted earnings came in at $2.58 per share, well above the consensus estimate of $1.85 and a 42.5% increase from the prior-year quarter. Revenue totaled $106.1 billion, up 7.3% year-over-year and roughly 6% higher than Wall Street had projected. Adjusted operating income surged 35.4% to $5.16 billion.

Guidance Boost Centered on Aetna

CVS raised its 2026 adjusted EPS outlook to a range of $7.90 to $8.10, up from its previous estimate of $7.30 to $7.50. The midpoint of $8.00 is 7.4% above the analyst consensus prior to the report. The company also increased its revenue minimum by $9 billion to $414 billion and lifted its operating cash flow minimum from $9.5 billion to $11.5 billion.

According to company calculations, the Health Care Benefits segment—which includes the Aetna insurance business—accounted for roughly 98% of the increase in consolidated operating profit at the midpoint. The segment's adjusted operating income guidance was raised by $1.03 billion at the midpoint, while the Health Services segment's minimum remained unchanged. The Pharmacy and Consumer Wellness segment saw its minimum profit raised by $220 million.

Q2 Segment Performance

Health Care Benefits posted an 85.5% rise in adjusted operating income to $2.43 billion, with the medical benefit ratio (MBR) improving by 250 basis points to 87.4%. The improvement was driven by pricing in government plans and tighter Medicare cost management. However, CVS noted that $500 million of the MBR outperformance came from favorable prior-year estimate adjustments, such as risk adjustment and favorable development, which limits the clarity of the ongoing run-rate.

Health Services continued to expand, with revenue up 11.5% to $51.8 billion and profit climbing 10% to $1.73 billion. Claims volume rose only about 1%, and CVS pointed to some profit being pulled forward from the second half, as well as ongoing challenges in the 340B drug pricing program.

Retail pharmacy performance remained stable, with revenue up 0.7%, prescription volume up 4.3%, and profit advancing 10.2%. CVS raised the full-year profit minimum for this segment by $220 million.

Strategic Moves and Market Context

CEO David Joyner stated that the businesses "continue to deliver strong performance," and the figures show that this performance translated into higher guidance at Aetna. The company also expanded its GLP-1 offerings in partnership with Eli Lilly (NYSE:LLY), introducing app-based pricing for same-day pickup for qualifying Zepbound and Foundayo patients, and reducing MinuteClinic virtual visit fees to $29. While this could boost customer footfall, it is not the primary earnings driver at present.

The results come amid heightened expectations across the health insurance sector, as UnitedHealth Group (NYSE:UNH) raised its outlook in July on improved medical cost control. CVS must now meet these elevated expectations, with medical utilization remaining high throughout the quarter.

Risks and Outlook

Looking ahead, repeatability is the key question. CVS factored a 75-basis-point MBR improvement into the midpoint of its yearly guidance, and the stock's rerating now depends on that metric rather than retail traffic. The company faces ongoing 340B challenges and broader macroeconomic pressures that could limit gains in the second half, but the strong quarter and raised guidance provide a solid foundation for the remainder of 2026.
