A study published on July 22 in Frontiers in Pediatrics examined 46 pediatric patients across 52 treatment courses at UPMC Children’s Hospital. Researchers used epoprostenol, a short-acting anticoagulant that reduces platelet aggregation, as the sole anticoagulant during Aquadex therapy. The median treatment duration was three days, with a filter-to-day ratio of 0.5—meaning approximately one filter was used every two days. Notably, no cases of bleeding were directly attributed to epoprostenol. However, 29% of treatment courses experienced at least one interruption due to clotting or clogging. The study was retrospective, single-site, and lacked a concurrent control group, which the authors noted limits the ability to draw causal conclusions.

Revenue and Financial Outlook

Nuwellis provided initial estimates for second-quarter revenue, projecting approximately $1.96 million. This would represent a 14% year-over-year increase from the $1.725 million reported in the second quarter of 2025, but an 18% sequential decline from the first quarter of 2026, which saw $2.4 million in revenue. For the first half of 2026, estimated revenue stands at roughly $4.35 million, up 20% from $3.629 million in the same period last year. The final, unaudited results are expected to be released on August 13. The company highlighted that these figures are preliminary and subject to revision.

Market Context and Challenges

Despite the stock’s explosive move, Nuwellis faces significant fundamental challenges. As of March 31, the company held only $2.2 million in cash and restricted cash, with no debt. It posted a net loss of $4.3 million in the first quarter. A public offering in June raised $6 million in gross proceeds. The company executed a 1-for-35 reverse stock split on June 25 and has shareholder authorization for another reverse split (ranging from 1-for-5 to 1-for-70) to maintain Nasdaq listing compliance, though the board has not yet declared a second split. The high trading volume—over 103 million shares—was 33.7 times the total of 3.06 million share-equivalents from recent issuances, indicating heavy speculation.

In the kidney-device sector, peer companies showed little movement. SeaStar Medical Holding Corp. (NASDAQ: ICU), which offers a pediatric acute kidney injury device, slipped 2.8%. Outset Medical, Inc. (NASDAQ: OM), maker of the Tablo dialysis system, gained 2.1%. Nuwellis plans to submit a 510(k) application before year-end to expand Aquadex approval to patients as light as five kilograms, down from the current 20-kilogram threshold. The company also reported its first pediatric system placements in Wisconsin on Tuesday.

Chief Executive Mike McCormick stated that the evidence is “essential to expanding confidence” in Aquadex. Senior author Dana Fuhrman described epoprostenol as “another tool at the bedside.” However, the study’s limitations—retrospective design, single site, no control group—mean that the findings are preliminary.
